Tag Applications Procedure (Active/Associate Member)

Make a copy of the registration, but make sure you are the owner or co-owner on the registration (State FOP Policy). Send the copy of the registration card along with a check for $15.00 to the Lodge Secretary (Mike Shaffer) or you may pay online at the below link. Be advised a $2.00 transaction fee will be required for online payment.  The secretary will send the applications to you. You may email him the copy of the registration to Secretary@mdfop34.org.
